Fotocopy Ijazah terakhir ( 2 lembar ) dan aslinya dibawa serta. &nbsp;&nbsp; &nbsp;&nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>3. Fotocopy nilai raport kelas terakhir SMA\/Sederajat dan Nem ( 2 lembar ) &nbsp;&nbsp; &nbsp;&nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>4. Pas Foto terbaru ukuran &nbsp;4 x 6 cm dan &nbsp;4 x 3 cm masing-masing ( 4 lembar ) &nbsp;&nbsp; &nbsp;&nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>5. Surat Baptis terbaru dan fotocopy ( 2 lembar ) &nbsp;&nbsp; &nbsp;&nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>6. Surat Rekomendasi dari Pastor Paroki. &nbsp;&nbsp; &nbsp;&nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>7. Surat Keterangan Berbadan Sehat dari Dokter ( 2 lembar ) &nbsp;&nbsp; &nbsp;&nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>8. Fotocopy Kartu Tanda Penduduk \/ Surat Domisili (2 lembar) &nbsp;&nbsp; &nbsp;&nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>9. Fotocopy Akta Kelahiran \/ Surat Keterangan Kenal Lahir (2 lembar) &nbsp;&nbsp; &nbsp;<\/p>\n\n\n\n<p>10.
